The Department of Medicine, HIV/AIDS Medical Care Coordination (MCC) program is seeking a Social Worker Associate to work under the guidance of our Medical Care Manager/Program Manager. In this role, you will serve as the Retention Outreach Specialist (ROS) and Case Worker. Accordingly, you will be responsible for developing and implementing targeted patient retention interventions, ensuring that our patients receive the highest quality care within an HIV Medical Home. By collaborating with the MCC team, you will help to locate and reach out to patients who have fallen out of HIV medical care, using your skills to engage and re-engage them in vital HIV care services. As an integral member of our Medical Care Coordination team, you will collaborate closely with your colleagues to identify, locate, and support patients in their journey towards better health outcomes. You will provide outreach services in the community and utilize public resources to provide the best possible care for our patients. In addition to your ROS responsibilities, you will also address patients' socioeconomic needs as a Case Worker, working closely with our Medical Care Manager and Program Manager to monitor patient progress, follow up on service linkages, and track outcomes. This position offers a unique opportunity to make a difference in the lives of patients living with HIV/AIDS, encompassing the roles of 50% Retention Outreach Specialist and 50% Case Worker. Target hourly rate: $28.78-$37.99

Qualifications

Required: • Demonstrates ability to conduct a manualized intervention program • Demonstrates ability to perform medical care retention in a non-judgmental manner • Demonstrates ability to understand and execute a complex health intervention system • Demonstrates ability to maintain relationships and develop networks of providers to encourage and retain PLWHA in HIV care • Ability to work and solve problems with minimal supervision. • Strong interpersonal communication skills to effectively and diplomatically interact with others. • Strong written and verbal communication skills. • Familiarity with AIDS symptoms and HIV medications. • Ability to deal with and maintain confidential and sensitive information with discretion and meet all HIPAA requirements. Ability to determine what information is confidential and to protect the confidentiality of that information. • Knowledge with HIV/AIDS-specific social services in Los Angeles County • Experience working with people living with HIV/AIDS
